i  don't  think  connecting  it  with  18  W  ballast  would  be  a  problem  apart  from  some  extra  heating  (that  too  wont  be  substantial  for  18  w,  don't  use  40  W  ballast  though).  recheck  the  wiring  if  its  not  working  with  that  ballast  else  there  is  somthin  wrong  with  your  tube.

u  can't  blow   up  the  tube  just  by  connect  it  with  the  ballast  which  is  rated  just  3W  above  the  required  value.  i  have  been  using  1  for  quite  a  long  time  without  any  problem.  may  be  somthin  is  wrong  with  the  connection  or  the  tube  or  the  ballast  itself.  posting  a  pic  may  be  helpful.

I  don't  think  pics  are  going  to  be  of  any  help  in  this  case.

Yes  you  can  definitely  run  a  15  W  tube  on  a  18W  ballast  without  any  problem.
I  had  done  the  same  without  any  issues  for  about  an  year  before  I  upgraded  to  40W  ones.

If  you  have  an  alternate  ballast,  check  the  tube.  In   most  cases,  the  tube  would  have  gone  dead.
Else  you  can  get  it  checked  form  a  local  electrical  stores.
